If you are registering a limb-different child aged 4-17, please use this option. Please note: One adult must register and remain on-site for each family with participating children under the age of 10. PAYMENT OPTIONS If you are requesting a scholarship, please do not complete this form until you have submitted a scholarship application (available at nubability.org) and received a scholarship code. An installment payment plan is available: 25% deposit upon registration and two additional payments.
