The Platinum 360 is offered for a limited time only and to a limited number of participants. Areas of focus: • First touch • Exploding into space • Dynamic play • High-level ball control • Skills under pressure • Possession & Distribution • Finishing • Competitive games played GUIDELINES: - The time and dates for professional personal training are to be decided upon the convenience of U.S.A and the participating parent. - U.S.A reserves the right to terminate or discontinue any part of or complete session at any time. - Either party is required to give at least 24 hours’ notice should there be any changes in schedule. - The package does not include U.S.A Elite Select Camp, Group Camps, and Semi-Private training. - The package expires one (1) year from the date of purchase. - There will be no refund. - Dates and times to be scheduled upon completion of registration. - U.S.A expects all registered participants to complete the training in its entirety. - Parents of participants must sign a waiver for his/ her child in order to participate in the U.S.A program. Universal Soccer Academy - Waiver, Release & Hold Harmless Agreement: I hereby give permission for Universal Soccer & Futsal Academy to secure proper treatment for my Son/Daughter. In the case of emergency, I do hold harmless, Universal Soccer Academy, it’s officers and employees, agents and contractors from any responsibilities for any and all injuries or death which may result from my participation in any programs offered by Universal Soccer Academy. I hereby agree to assume all the liability and risk from participating in any Soccer Programs offered
