Description
Your child’s coach will notify you about team practices and other important information. You must list your correct email address and phone # when registering online or in person.
Games will be played on Sat mornings beginning Dec 12 - Feb 20,
Picture day is scheduled for Saturday, Jan 16.
Please let us know ASAP if you can coach your son or daughter's team.
Every player must attend the player assessment program. All students registered will be placed on a team. The player assessment program allows coaches to evaluate players' skills to ensure equitable team drafting.
Player Assessments will take place on Saturday, October 24, 2026
3rd & 4th Graders' assessment is 8:00 - 8:45 am, High School Fieldhouse
5th & 6th Graders' assessment is 9:30 - 10:15 am, High School Fieldhouse
7th & 8th Graders' assessment is 10:45 - 11:30 am, High School Fieldhouse
A youth-sized t-shirt is included with your registration fee
A Parent/Guardian must initial for anyone under 18 years old. Permission is granted for my son/daughter to participate in the above activity as it was discussed in agreement with the South Milwaukee Recreation Department.
I understand that neither the South Milwaukee Recreation Department assumes liability for payment of medical-hospital expenses, which may be incurred by myself or the person on this form while participating in any activity, but said I would assume liability for the duration of the activity. If a health condition exists that would limit participation in this activity, I will notify the South Milwaukee Reaction Department.
I agree to the unreserved use of my name and/or likeness (including photographs) for publicizing South Milwaukee Recreation Department Programs. We may photograph several of our activities and their participants for program promotion purposes.
