JOIN THE SMMS WRESTLING TEAM
This program is for boys and girls in 6th, 7th & 8th grade. The team will meet from 3:30-5:30 pm in the Middle School Multipurpose Room Enter through door #34
Monday, Thursday & Friday Dec 15-18.
Practice over Winter Break: Date & Times TBD
Monday, Thursday & Friday Jan 5–March 12.
Pay Via Rec Dept: Activity Code: XCRR202.202, fee $120
Parent/Guardian must initial for anyone under 18 years old. Permission is granted for my son/daughter to participate in the above activity as it was discussed in agreement with the South Milwaukee Recreation Department. I understand that neither the South Milwaukee School Board nor the South Milwaukee Recreation Department assumes liability for payment of medical-hospital expenses, which may be incurred by myself or the person on this form while participating in any activity, but said liability will be assumed by me for the duration of the activity. If a health condition exists that would limit participation in this activity, I will notify the South Milwaukee Recreation Department.I agree to the unreserved use of my name and/or likeness (including photographs) for publicizing South Milwaukee Recreation Department Programs. We may photograph several of our activities and their participants for program promotion. Photos may be used in various flyers, brochures, or our website. Please let the photographer know if you want to avoid having your photo taken.
