How are they differ- ent and yet the same as you?So, let’s travel to: IRELAND and dance like a Leprechaun! BOLIVIA-to be in their Carnival Parade (with your hand- made mask.) INDIA for fun games Indian children play...”chipko” anyone? POLAND-Make an obwarzanek (Polish bagel) and dance the polka. PERU to grind grain with a real metate and have fun doing the”Llama Weaving Dance.”
Parent/Guardian must initial for anyone under 18 years old. Permission is granted for my son/daughter to participate in the above activity as it was discussed in agreement with the South Milwaukee Recreation Department. I understand that neither the South Milwaukee School Board nor the South Milwaukee Recreation Department assumes liability for payment of medical-hospital expenses, which may be incurred by myself or the person on this form while participating in any activity, but said I would assume liability for the duration of the activity. If a health condition exists that would limit participation in this activity, I will notify the South Milwaukee Recreation Department. I agree to the unreserved use of my name and/or likeness (including photographs) for publicizing South Milwaukee Recreation Department Programs. We may photograph several of our activities and their participants for program promotion. Photos may be used in flyers, brochures, or our website. Please notify the photographer if you wish to avoid having your photo taken.
